I have been in practice since 2014 treating individuals of all ages and families with a speciality in working with children and in perinatal mental health. I am a Licensed Clinical Social Worker with Registered Play Therapist-Supervisor and Perinatal Mental Health-Certified credentials holding licensures to practice in Indiana and Wisconsin. I enjoy working in collaboration with all patients for a positive therapeutic experience. I help empower you to live your most joyful life. You are the expert and it's my role to walk alongside and guide you as you meet your treatment goals.
I earned my BA Degree in Psychology and continued my education at the Indiana University School of Social Work where I earned my Master's of Social Work (MSW). My graduate work was in the fields of domestic violence prevention and advocacy, sexual assault prevention and advocacy, crimes against children prevention and advocacy, and medical social work. Post-graduate work includes medical social work, crisis management, individual and group supervision, school-based therapy, individual and group outpatient psychotherapy including play therapy, and teaching at the associate and graduate levels. I am proud to be a social worker! Areas of continued work and professional interest outside of therapy include medical ethics, clinical supervision, and teaching.

Play Therapy is defined by the Association for Play Therapy as “the systematic use of a theoretical model to establish an interpersonal process wherein trained Play Therapists use the therapeutic powers of play to help clients prevent or resolve psychosocial difficulties and achieve optimal growth and development."
Play is the language of children and it is our speciality. The right combination for talk and play therapy is important to best support children's mental health.

Postpartum depression, anxiety, and other mental health conditions occurring in during pregnancy and up to a year following are not commonly talked about but do commonly effect new parents. Care and treatment from a empathic and knowledgable provider can make a big difference.

In addition to individual and family therapy, Myers Mental Health offers supervision to those with an Licensed Social Worker (LSW) licensure seeking Licensed Clinical Social Worker (LCSW) licensure and supervision for any independently licensed mental health professionals seeking Registered Play Therapist (RPT) or Registered Play Therapist-Supervisor (RPT-S) credentialing. Consultation services are available for seasoned for professionals and those looking for social work or play therapy collaboration. Supervision and consultation are offered on a sliding scale basis with lower fees available for pre-licensed professionals and community mental health workers. Please inquire by message.
Myers Mental Health is located at 148 Sagamore Parkway West in West Lafayette, IN serving children, adults, and families the Greater Lafayette/West Lafayette area. We also offer telehealth services for older teens and adults anywhere in the state of Indiana or Wisconsin.
Treatment will start with a free, 15 minute phone consultation to see if we are a good fit to work together. Once we decide to start treatment together, you will receive a link to the patient portal where you will complete intake paperwork and sign consents. Once this has been completed, you will come to your first session. The first appointment for a child or adolescent patient will always be a parent/caregiver session. This will allow us to review a full history and plan for goals of treatment for your child. You can expect to feel heard, supported, and empowered to start your treatment journey.
The frequency of therapy sessions will depend on your individual needs and goals. Many clients attend therapy once a week, while others may attend less frequently. We will work with you to develop a personalized treatment plan. For pediatric patients, treatment will include a combination of individual and family therapy sessions. We keep caregivers involved every step of the way! Once the initial caregiver session is complete, it is common to have several sessions with a child/teen before having another session with the caregiver. Caregivers can reach out anytime during the treatment process and request a family or caregiver session.
Myers Mental Health, LLC is OON with all insurance providers and can provide you a superbill so you can maximize access to your OON benefits. Therapy sessions are between 50-60 minutes in length. The price per session is $135. The initial evaluation appointment including the biopsychosocial assessment is 60 minutes and is $200. Therapy is an eligible healthcare expense and HSA and FSA funds can be used to pay for your treatment.

Myers Mental Health, LLC (the “Practice”) is committed to protecting your privacy. The Practice is required by federal law to maintain the privacy of Protected Health Information (“PHI”), which is information that identifies or could be used to identify you. The Practice is required to provide you with this Notice of Privacy Practices (this “Notice”), which explains the Practice's legal duties and privacy practices and your rights regarding PHI that we collect and maintain.
YOUR RIGHTS
Your rights regarding PHI are explained below. To exercise these rights, please submit a written request to the Practice at the address noted below.
To inspect and copy PHI.
• You can ask for an electronic or paper copy of PHI. The Practice may charge you a reasonable fee.
• The Practice may deny your request if it believes the disclosure will endanger your life or another person's life. You may have a right to have this decision reviewed.
To amend PHI.
• You can ask to correct PHI you believe is incorrect or incomplete. The Practice may require you to make your request in writing and provide a reason for the request.
• The Practice may deny your request. The Practice will send a written explanation for the denial and allow you to submit a written statement of disagreement.
To request confidential communications.
• You can ask the Practice to contact you in a specific way. The Practice will say “yes” to all reasonable requests.
To limit what is used or shared.
• You can ask the Practice not to use or share PHI for treatment, payment, or business operations. The Practice is not required to agree if it would affect your care.
• If you pay for a service or health care item out-of-pocket in full, you can ask the Practice not to share PHI with your health insurer.
To obtain a list of those with whom your PHI has been shared.
• You can ask for a list, called an accounting, of the times your health information has been shared. You can receive one accounting every 12 months at no charge, but you may be charged a reasonable fee if you ask for one more frequently.
To receive a copy of this Notice.
• You can ask for a paper copy of this Notice, even if you agreed to receive the Notice electronically.
To choose someone to act for you.
• If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights.

OUR USES AND DISCLOSURES
1. Routine Uses and Disclosures of PHI
The Practice is permitted under federal law to use and disclose PHI, without your written authorization, for certain routine uses and disclosures, such as those made for treatment, payment, and the operation of our business. The Practice typically uses or shares your health information in the following ways:
To treat you.
• The Practice can use and share PHI with other professionals who are treating you or to support in the treatment of you.
• Examples: Your primary care doctor asks about your mental health treatment. Your therapist participates in peer supervision to continue to maintain best practices in your care.
To run the health care operations.
• The Practice can use and share PHI to run the business, improve your care, and contact you.
• Example: The Practice uses PHI to send you appointment reminders if you choose.
To bill for your services.
• The Practice can use and share PHI to bill and get payment from health plans or other entities.
• Example: The Practice gives PHI to your health insurance plan so it will pay for your services. Your provider is an out-of-network (OON) provider with all insurance companies.
2. Uses and Disclosures of PHI That May Be Made Without Your Authorization or Opportunity to Object
The Practice may use or disclose PHI without your authorization or an opportunity for you to object, including:
To help with public health and safety issues
• Public health: To prevent the spread of disease, assist in product recalls, and report adverse reactions to medication.
• Required by the Secretary of Health and Human Services: We may be required to disclose your PHI to the Secretary of Health and Human Services to investigate or determine our compliance with the requirements of the final rule on Standards for Privacy of Individually Identifiable Health Information.
• Health oversight: For audits, investigations, and inspections by government agencies that oversee the health care system, government benefit programs, other government regulatory programs, and civil rights laws.
• Serious threat to health or safety: To prevent a serious and imminent threat.
• Abuse or Neglect: To report abuse, neglect, or domestic violence.
To comply with law, law enforcement, or other government requests
• Required by law: If required by federal, state or local law.
• Judicial and administrative proceedings: To respond to a court order, subpoena, or discovery request.
• Law enforcement: For law locate and identify you or disclose information about a victim of a crime.
• Specialized Government Functions: For military or national security concerns, including intelligence, protective services for heads of state, or your security clearance.
• National security and intelligence activities: For intelligence, counterintelligence, protection of the President, other authorized persons or foreign heads of state, for purpose of determining your own security clearance and other national security activities authorized by law.
• Workers' Compensation: To comply with workers' compensation laws or support claims.
To comply with other requests
• Coroners and Funeral Directors: To perform their legally authorized duties.
• Organ Donation: For organ donation or transplantation.
• Research: For research that has been approved by an institutional review board.
• Inmates: The Practice created or received your PHI in the course of providing care.
• Business Associates: To organizations that perform functions, activities or services on our behalf.
3. Uses and Disclosures of PHI That May Be Made With Your Authorization or Opportunity to Object
Unless you object, the Practice may disclose PHI:
If it is in your best interest because you are unable to state your preference.
4. Uses and Disclosures of PHI Based Upon Your Written Authorization
Myers Mental Health, LLC will never market or sell personal information.
OUR RESPONSIBILITIES
• The Practice is required by law to maintain the privacy and security of PHI.
• The Practice is required to abide by the terms of this Notice currently in effect. Where more stringent state or federal law governs PHI, the Practice will abide by the more stringent law.
• The Practice reserves the right to amend Notice. All changes are applicable to PHI collected and maintained by the Practice. Should the Practice make changes, you may obtain a revised Notice by requesting a copy from the Practice, or using the information above.
• The Practice will inform you if PHI is compromised in a breach.

You are entitled to receive this Good Faith Estimate of what the charges could be for
psychotherapy services provided to you upon entering into treatment at Myers Mental Health. While it is not possible for a clinical social worker to know, in advance, how many psychotherapy sessions may be necessary or appropriate for a given person, a form that provides an estimate of the cost of services will be provided to you. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you. This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service.
This estimate is not a contract and does not obligate you to obtain any services from the provider(s) listed, nor does it include any services rendered to you that are not identified on the form. The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. There may be additional items or services I may recommend as part of your care that must be scheduled or requested separately and are not reflected in this Good Faith Estimate. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill. You have the right to initiate a dispute resolution process if the actual amount charged to you substantially exceeds the estimated charges stated in your Good Faith Estimate (which means $400 or more beyond the estimated charges). You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available. You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill. There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount. I anticipate your treatment may require bi-weekly 55-minute psychotherapy sessions
throughout the next 12 months at $135 per session for a total of 26 taking into consideration availability for an estimated total of $135/session x 26 weeks. Based upon a fee of $135
per visit, if you attend one (psychotherapy) session per 2 weeks, your estimated charge
would be $_3,510__for 26 visits over one year. Estimated charges will increase according to the
number of session and length of treatment. This Good Faith Estimate is not intended to
serve as a recommendation for treatment or a precaution that you may need to attend a
specified number of psychotherapy visits. The number of visits that are appropriate in your
case, and the estimated cost for those services, depends on your needs and what you agree to in consultation with your therapist. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time.
You are encouraged to speak with your provider at any time about any questions you may have
regarding your treatment plan, or the information provided to you in this Good Faith
Estimate.

-Legal Fees, $200/hr. All services related to court involvement, including court testimony, time spent reviewing records and preparing, documents, travel time, and communications
with parents’ legal counsel and/or with Guardian ad Litems. Court appearances are protected in half-day (four hour) increments. The minimum fee of $800 for the first half day of court time is due prior to the date of the court appearance. If more time is needed, the fees will be prorated in fifteen minute increments based on the Legal Fees above.
